Ring the Bell for Mental Health

A while back I ended a column on what I perceived as the insanity of my daily “news” feed on social media noting that one post had informed me that May was Mental Health Awareness Month. “Sign me up,” I wrote in an attempt to end a column on an unfunny state of affairs with a bit of levity. A writer’s gimmick.

Now I feel a need to clarify. I don’t think mental health is a joking matter and, in fact, I’ve already signed up.

On May 1, I had the honor of reading The History of the Bell at the annual meeting of Mental Health Association in Orange County (N.Y.). The honor was mine as the outgoing president of the board of directors of the private, non-profit agency, a post I was privileged to hold for five years.

I’m including that history here because I believe it deserves a broader audience.

The History of the MHA Bell

During the early days of mental health treatment, asylums often restrained persons with mental illnesses with iron chains and shackles around their ankles and wrists. Clifford Beers, the founder of the Mental Health Association movement, experienced and witnessed many of these and other abuses. After his own recovery, he became a leading figure in the movement to reform the treatment of, and attitudes toward, mental illness. With better understanding and treatment, cruel practices eventually stopped.

In the early 1950s, in the lobby of the National Headquarters in New York City, the Mental Health Association collected discarded chains and shackles from asylums across the country. All of these restraints were then shipped to the McShane Bell Foundry in Baltimore, Maryland, where they were dropped into a crucible and cast into a 300-pound bell.

The inscription on the bell reads:

“Cast from shackles which bound them,
this bell shall ring out hope for the mentally ill
and victory over mental illness.”

As we seek the vision of victory over mental illness, we need the participation of all citizens in shaping the future of mental health services. We need to remove the shackles from the wisdom of recipients of mental health services and their families and recognize the value of their experience in shaping future policy. Through full citizen participation,

This bell will ring for hope,
This bell will ring for freedom,
This bell will ring for victory.

We’re big in this country on designating special months and days for special causes, but I don’t think mental health awareness should go away on June 1 for another year.

Not in a time when those with the power to return us to the years of chains and shackles are talking about slashing or eliminating financial support for agencies like MHA and thousands of others that were called upon to provide the care and assistance needed by those released when those oppressive asylums were closed down across the country.

Not in a time when persons without proper health training, experience and credentials are in charge of national health policy.

Not in a time when Congress is discussing eliminating Medicaid, which pays for much of that support to thousands of Americans.

Not in a time when thousands of federal employees who provide research and other vital assistance to those suffering with mental illness are being let go.

Not in a time when when federal funding for agencies that address addiction problems is being eliminated.

Not in a time when programs for veterans dealing with mental illness are being eliminated.

Not in a time when politicians are talking about eliminating the national suicide hotline.

Not in a time when anyone who doesn’t fit the powers-that-be’s increasingly narrow profile of “acceptable” is in danger of being snatched off the street and placed in shackles.

May is almost over, but the bell for hope and freedom for the mentally ill needs to keep ringing. Mental health is health. Sign me up.

   Perhaps lost to the public in the daily onslaught of massive cuts to the workforces of federal agencies is the 10 percent reduction, with the threat of possibly another 50 percent to come in one of the smaller, but not unimportant, agencies: SAMHSA.

    SAMHSA stands for Substance Abuse and Mental Health Services Administration. It is part of the federal government’s health network. Its mission:  “To lead public health and service delivery efforts that promote mental health, prevent substance misuse, and provide treatment and support to foster recovery while ensuring equitable access and better outcomes.” It used to say for all.

     Like other federal agencies, SAMHSA employees say they can’t effectively deliver on their mission without sufficient resources.  Among the services provided to local mental health agencies and local governments are the 24-hour suicide hotline and information and resources to deal with drug addiction and alcoholism.

      Seeing as the new health commissioner, Bobby Kennedy Jr., has a history of drug addiction, and tea-totaler Donald Trump has an oft-stated fear of alcohol and alcoholism, having watched his brother Fred die in his 40s of the disease, it’s surprising to me that they would want to weaken this particular agency. 

   Then again, maybe Elon Musk didn’t tell them about these cuts and resents Kennedy reviving studies to try to tie the use of vaccines to autism, a theory long disproved.

    In any event, one of the useful services provided by SAMHSA is a self-test to determine if you have a problem with alcohol. I’ve run the test a number of times in this column and it seems to be an appropriate time to do it again. It’s called AUDIT.

       AUDIT stands for alcohol use disorders identification test. It’s the most widely used test in the world and is compiled by the World Health Organization. Shhh. Don’t tell Trump.

       And be honest.

Scoring the AUDIT

Scores for each question range from 0 to 4, with the first response for each question (eg never) scoring 0, the second (eg less than monthly) scoring 1, the third (eg monthly) scoring 2, the fourth (eg weekly) scoring 3, and the last response (eg. daily or almost daily) scoring 4. For questions 9 and 10, which only have three responses, the scoring is 0, 2 and 4.

The range of possible scores is from 0 to 40, with 0 indicating an abstainer who has never had any problems from alcohol. A score of 1 to 7 suggests low-risk consumptions, according to World Health Organization (WHO) guidelines. Scores from 8 to 14 suggest hazardous or harmful alcohol consumption and a score of 15 or more indicates the likelihood of alcohol dependence (moderate-severe alcohol use disorder).

If your score concerns you, there is help available. Talk with your primary care doctor. Call an Alcoholics Anonymous hotline in your area. Call a mental health crisis hotline if one is available.

   Excessive alcohol use is now classified as a mental disorder by health officials, somewhat in response to the stigma attached to the word “alcoholic.”  However, most recovering members of Alcoholics Anonymous have no problem with identifying as such. In any case, it is not a moral failing. It can be treated. First, it has to be acknowledged.

    Again, be honest.

     There has been plenty of news coverage of the daily stream of complaints from the twice-impeached, four times indicted former president that (1.) accusing him of crimes (91 of them) for things he has said and trying to silence him from talking about the accusations constitute an attack on his First Amendment right of free speech, but (2.) the most recent legitimate threat to the First Amendment has received much less attention, possibly because it happened in Marion, Kansas, where the entire sheriff’s force raided the offices of the local paper, the Marion County Record, and the home of its owners, taking computers, phones, notebooks, etc., looking for the source of information on embarrassing news about a local politician and a business owner, even though the paper had not published articles on either person and despite a warrant that the local DA invalidated two days after the raid as unwarranted, leading (3.) news media organizations to denounce the rare government interference in the operation of a free press, an action which the editor said (4.) created stress which contributed to the death of his 98-year-old mother and newspaper co-owner a day after the raid, which is tragic, as is each of (5.) the estimated 49,500 people who committed suicide in the United States last year, the highest number ever, according to a report from the Centers for Disease Control, which said suicides had become more commmon in America than any period since just before World War II, a war whose outcome established the former Soviet Union as a world power, to fear and grudgingly respect, both of which were absent as (6.) Ukraine continued its counteroffensive in the disastrous war launched against it by Russian President Vladimir Putin, which revealed the weakness of Russia’s military, and (7.) Russia’s robotic Luna-25 spacecraft crashed on the surface of the moon, much as (8.) Hunter Biden’s plea deal on tax fraud and gun charges did when a judge

refused to accept the terms and (9.) Britain’s hopes of magically winning a World Cup in women’s soccer did when gritty Spain won the title match, one-nothing, which (10.) experts said was pretty much what Hawaii had done to prepare for the disastrous wildfire that devastated Maui, leveling a town and killing more than a hundred people, the kind of devastation Democrats could experience in 2024 if (11.) the deceptively named No Labels Party runs a candidate for president, since the moderate-conservative group wouldn’t take away any of Trump’s loyal followers (assuming he’s not in prison), but could sway some independents away from voting for Joe Biden, who (12.) practiced statesmanship by hosting the leaders of Japan and South Korea, traditional rivals if not enemies, at Camp David, to forge an alliance in the three countries’ favor, kind of the opposite approach of Trump, who (13.) said he would skip the scheduled Republican presidential candidates debate in favor of an interview with Tucker Carlson somewhere Trump can presumably demonstrate his right to free speech ad nauseum without fear of someone confronting him with facts, kind of like (14.) Rudy Giuliani‘s approach claiming that the RICO law, which he is charged with violating in trying to overturn the 2020 presidential election results in Georgia, does not apply to conspiracies among political  figures, even though, as the first U.S. prosecutor to use the law, Giuliani, who (15.) has experienced an epic fall from 9/11 grace, (16.) once bragged how he used it against Mayor Ed Koch and other New York City political figures.

Teens, Food, Eating … Addiction?

73602518-9FFF-4EBA-93AC-F0BDAB457FB1The list of substances or behaviors to which adolescents can become unhealthily attached, even addicted, can seem endless to a parent inclined to be caring and protective: alcohol, smoking, drugs, gambling, video games, social media, internet, cell phones, sex, shopping, eating …

Wait, eating you say? Yes, eating. Or food. It depends. There’s a debate over whether the problems are the same thing. Some say that someone who craves the same food, say sweets or salty chips, and consumes it in unhealthy amounts might be considered a food addict, a term not universally accepted, but one that is useful in defining a behavior. The food in question reacts on the brain in the same way that alcohol or another drug would. It rewards the person, who feels good.

Some say that someone for whom eating — anything and plenty of it — is a fulltime job with significant negative consequences might be considered to have an eating addiction, rather than a food addiction. Eating may provide the same kind of escape and temporary excitement that gambling, for example, would in someone else. An irresistible reward.

And, of course, these harmful behaviors often co-exist. Addictions may have biological, psychological, or social causes, or, likely, a combination of them.The focus here is not on debating the food/eating addiction question, but rather on recognizing that food addictions and eating disorders — a different category of self-destructive behavior, including anorexia, bulimia and binge eating — can often be ignored in teens when there is so much talk in media — social and otherwise — about drinking and driving, opioid abuse, the pros and cons of marijuana and the rest of that list.

Teens eat, adults say. Sometimes they eat a lot. Maybe a lot of junk food. They’re growing. So what’s the big deal?

Maybe nothing; maybe something. A primary goal of this column is to provide useful information to help readers identify and get help for addictive behavior and some studies say up to 10 percent of Americans may have a food-related addiction or disorder. One in 10 female teens may have an eating disorder. A smaller percentage of teen boys, perhaps athletes such as wrestlers or runners, also have issues that revolve around body weight and image. What follows are some symptoms and questions to help you decide if you or someone you know, perhaps a teenager, has a health issue involving food. 

The following are possible symptoms of a food addiction:

— Gorging

— Eating to the point of feeling ill

— Going out of your way to obtain certain foods

— Continuing to eat certain foods even if no longer hungry

— Eating in secret

— Avoiding social interactions, relationships to spend time eating certain foods.

— Difficulty functioning in a job or school due to decreased efficiency

— Spending a significant amount of money on buying certain foods to binge

— Obesity

— Fatigue

— Difficulty concentrating

— Sleep disorders, such as insomnia or oversleeping

— Headaches

— Irritability

— Digestive disorders

— Thoughts of suicide

Food Addicts in Recovery Anonymous, a 12-step group that seeks to help people with food addictions or disorders, offers its own quiz to help people determine if they have a problem with food. As always, answer as honestly as possible:

Have you ever wanted to stop eating and found you just couldn’t? 

Do you think about food or your weight constantly? 

Do you find yourself attempting one diet or food plan after another, with no lasting success? 

Do you binge and then “get rid of the binge” through vomiting, exercise, laxatives, or other forms of purging? 

Do you eat differently in private than you do in front of other people? 

Has a doctor or family member ever approached you with concern about your eating habits or weight? 

Do you eat large quantities of food at one time (binge)? 

Is your weight problem due to your “nibbling” all day long? 

Do you eat to escape from your feelings? 

Do you eat when you’re not hungry? 

Have you ever discarded food, only to retrieve and eat it later? 

Do you eat in secret? 

Do you fast or severely restrict your food intake? 

Have you ever stolen other people’s food? 

Have you ever hidden food to make sure you have “enough”?

Do you feel driven to exercise excessively to control your weight? 

Do you obsessively calculate the calories you’ve burned against the calories you’ve eaten? 

Do you frequently feel guilty or ashamed about what you’ve eaten? 

Are you waiting for your life to begin “when you lose the weight”?

Do you feel hopeless about your relationship with food?

A “yes” answer to any question could indicate a problem with food, the group says.

 Obviously, these are complex issues with serious — even life-threatening — potential consequences that need to be addressed as early as possible by trained professionals. There are a variety of programs and organizations to turn to If you suspect a food-related problem. Consult your doctor to begin with and check any of the accompanying links for more information.
